Please complete all forms prior to your initial appointment. Inevitably it will take some time to get to know one another. Please complete the forms as honestly as you can so we can get started, and you can always make adjustments as we learn more about the issues bringing you here.
We are located on the second floor, waiting room on the right, and unfortunately do not have an elevator. We do not always have front desk staff, so come on in and take a seat. We will be with you for your appointment.
